Bah Humbug. RUST!

Only just found (whilst cleaning) that what I thought to be a large tar blob is in fact an area of rust half way along N/S sill ) directly below the door rear shut line, which has bubbled up under the black anti-stone chipping paint (rev1). Had a closer look and poke about. Now found a rusted hole about 25mm x 10mm.
Two points:
i.How the blazes does this area become rusted from inside (did have very rear most section of sill filled after small dent, 12 years back)? Is it blocked drainage holes, as they appear to be ok?
ii.How do I rectify with a permanent solution. i.e. is it worth patching or a new sill? And any peeps have any idea of pricing?
Any recommendations for bodyshops in the S. Glos/N.Wilts area?
Worst part was I only just noticed having just purchased a excellent condition engine lid, after paint peeled from the original (under the grills).
