[Mk2] [Turbo] Rear brake pad dropped out. One for experts

Hi all,
Got a bit of an unusual problem - rear brake started to sound metal on metal so I naturally thought wear indicators were rubbing.
When I checked it out the inner pad had come out of its guides and was sat at an angle and had worn away into a wedge shape.
I'm running yellow stuff pads on oem discs. Calipers were replaced 2 years ago and pads were last done about 9months ago Handbrake works fine and car went thru MOT a month ago - caliper winds back ok and sliders seem fine. I was wondering if the dowel in the back of the pad might be too short.
Any ideas?

assuming the dowel on the pad is correctly located in the piston cutout then id start with measuring the thickness of the brake disc

Disc is 21.6 mm ( Rev 2)
My BGB is for a Rev 1 which says 15 mm anyone know what a Rev 2 minimum is?
I compared the yellow stuff to a Pagid and if anything the dowel is shorter on the Pagid so I think I can rule out the pads.

Perhaps the pads weren't fitted into the spring clips correctly. I wouldn't risk a repeat as it could completely seize a wheel, I'd fit new spring clips and OEM pads.

Yes, never use anything made by EBC, their brakes are crap.

Fit Porterfield pads, best brakes you will ever have.
