The memberlist shows inactive members as well. So all the ones that joined yesterday afternoon or today will feature in that list, but be unable to post since we now have manual account activation.
The problem is the 20,000 fake accounts registered before yesterday

I reckon I've nailed this spam thing now. If someone tries to register with an email address that's a known spam account (according to stopforumspam.com's list) they get:
instead of being able to register. I've seen no approval emails since putting this in place (as opposed to 1 new account every 10 minutes before), but I have been able to register a test account just fine using a proper email address.
I've also written a script to test the email address of each registered user against the same database, and disable the account if it finds a match.

Just clearing out all the accounts John's script has identified as using email accounts known to be in use with spammers.
