Toyota still can get a lot of trim peices, sure some will have been discontinued but the only way to really find out would be to ask them. The word trim covers
check where the exhaust hangers are mounted, its not uncommon for them to start splitting at thise points and cause a bit of a noise. Bit of welding and all was good with the last one,

I wouldn't say its beyond repair, you'll just have to treat it as a fairly intensive welding project.

I'd check for a vacuum leak. The hard metal vac pipe around the intake is well known to split and I know for a fact when it went on mine the car didn't drive right and as with your fuel consupmption it sounds about the same as I was getting.
